Ιστορική για τη σκληρότητά της έχει μείνει η σφαγή 7.000 Θεσσαλονικέων στον Ιππόδρομο το 390μ.Χ., με διαταγή του Θεοδοσίου, ως τιμωρία για την εξέγερση εναντίον της φρουράς του, που αποτελούνταν από Γότθους υπό τον Βουτέριχο.
Το 390 μ.Χ. ο διοικητής της γοτθικής φρουράς της Θεσσαλονίκης Βουτέριχος συνέλαβε και φυλάκισε, για ασήμαντο λόγο, έναν πολύ δημοφιλή ηνίοχο του ιπποδρόμου. Επειδή εκείνες τις μέρες θα γίνονταν αρματοδρομίες, οι Θεσσαλονικείς ζήτησαν από το Βουτέριχο να αφήσει ελεύθερο τον ηνίοχο, για να πάρει κι αυτός μέρος στους αγώνες. Ο Γότθος διοικητής όμως αρνήθηκε.
Η άρνηση αυτή στάθηκε αφορμή για να ξεσπάσει η δυσαρέσκεια και να πυροδοτήσει μια λαϊκή εξέγερση που ήταν φαινόμενο όχι ασυνήθιστο στο Βυζάντιο. Ο λαός της πόλης εξεγέρθηκε και στη διάρκεια των βίαιων ταραχών που ακολούθησαν, σκοτώθηκαν όλοι σχεδόν οι Γότθοι της φρουράς, καθώς και άλλοι αξιωματούχοι, ενώ πολλά δημόσια κτίρια καταστράφηκαν.
Όταν ο Θεοδόσιος πληροφορήθηκε τα γεγονότα εξοργίστηκε και αποφάσισε να τιμωρήσει σκληρά την πόλη. Στα Μεδιόλανα όπου βρισκόταν, ο επίσκοπος Αμβρόσιος προσπάθησε να τον πείσει να φανεί επιεικής, χωρίς αποτέλεσμα.
Η βάρβαρη γοτθική φρουρά της πόλης εκτέλεσε την τιμωρία με σατανικό τρόπο. Την επόμενη μέρα ο λαός προσκλήθηκε στον Ιππόδρομο, τάχα για να παρακολουθήσει τους αγώνες.. Κι όταν το ανυποψίαστο πλήθος συγκεντρώθηκε και παγιδεύτηκε, ξεχύθηκαν μέσα στον ιππόδρομο οι Γότθοι στρατιώτες και άρχισαν να σφάζουν όποιον έβρισκαν μπροστά τους, χωρίς διάκριση. Τα θύματα της φοβερής αυτής σφαγής έφτασαν τις 7.000 ή, κατ’ άλλους, τις 15.000.
Ύστερα απὸ το έγκλημα αυτό, ο Θεοδόσιος κατὰ την εορτὴ των Χριστουγέννων βρέθηκε στὰ Μεδιόλανα και θέλησε να μεταβεί στην Εκκλησία και να κοινωνήσει. Ο άγιος Αμβρόσιος, ντυμένος τα ιερατικά του άμφια, βγήκε στη θύρα του ναού και δεν επέτρεψε στον αυτοκράτορα να προχώρησει. «Είναι τα χέρια σου βαμμένα με αίμα», του είπε. Ο Θεοδόσιος θέλησε να δικαιολογηθεί κι απάντησε στον Ιεράρχη: «Κι ο Δαβὶδ αμάρτησε».
Τότε ο άγιος Αμβρόσιος είπε στον Θεοδόσιο. «Τον μιμήθηκες στο έγκλημα, να τον μιμηθείς και στη μετάνοια»!
Ο Θεοδόσιος δεν τόλμησε να μπει στην Εκκλησία κι έφυγε μετανιωμένος.
